When posting a finished shoutcast, title it as such
[Game Title] Requested by (user, request date) [Length]
for example
[League of Legends] Requested by bpoli19, 11/27/14 [35:21]
Downloading footage:
Some footage will be easily downloadable via the link posted, but if someone uses a youtube video, there are many options for downloading it. I prefer using keepvid.com to download youtube videos; just be aware that sometimes a high-quality video will have to be downloaded without the audio when using that site.
What should my video include?
Well at the bare minimum, commentary. All you need is your best broadcast of commentary over the video. This can easily be achieved by recording a separate audio file while watching the video and syncing up the video and audio in a simple video editing program and uploading the result to youtube. The rest is up to you; sometimes people will include their own replay in the video; edit that in with some graphics or transitions. Its open to how big and fancy you want to make it. Nothing is off limits.
